LIMITED FEDERAL DISCLOSURE STATEMENT FOR CHANGE IN TERMS OF THE ELECTRONIC SERVICES AGREEMENT
EFFECTIVE: MARCH 8, 2021
This limited disclosure is given in conjunction with the terms of F&A Federal Credit Union’s Electronic Services Agreement.
It is given by F&A Federal Credit Union in compliance with the Electronic Fund Transfer Act and Regulation E. This is not a complete Electronic Services Agreement but is limited to the change in terms of F&A Federal Credit Union’s Electronic Services Agreement.

Effective MARCH 8, 2021 the following changes will be made to your Electronic Services Agreement.
How to Notify the Credit Union in the Event of an Unauthorized Transaction
If you believe your checks, debit card and/or PIN have been lost or stolen or that someone will or may use it to transfer money from your account(s) without your permission, you must telephone us at: (800) 222-1226 during business hours.
Effective March 8, 2021, a new after-hours toll-free number to contact us can be found on our website.
You may also write us at: F&A Federal Credit Union, P.O. Box 30831, Los Angeles, CA 90030-0831.
In addition, you may report debit or credit card fraud in Online or Mobile Banking by clicking on Online Services and Report Card Fraud. You should also call the number or write to the address listed above if you believe a transfer has been made using the information from your check without your permission.
To help protect against unauthorized use, you may lock/unlock your card in Online or Mobile Banking.
